Welcome to the Schertz-Cibolo-Universal City Independent School District.  The Human Resources Department would like all new SCUC Employees to print and complete all forms listed below, in order to expedite processing during the New Hire Orientation sessions.  Please begin by printing the New Hire Forms Checklist, and use this as a guide for completing the remainder of your paperwork.

It is important to keep your forms in the order listed here.  If you have any questions on filling out a particular form, please leave it blank and we will help you complete the form at orientation.

Item I
Employee Information: Complete top portion only of the Employee Information form.
Item II
Form I-9: Complete Page 3, Section 1, sign and date.  A copy of your social security card and driver's license is required.
Item III
Form W-4: Complete Items#1-7, sign and date.
Item IV
Electronic Funds: Complete, sign, date and attach a voided check for the direct deposit of your pay semi-monthly.  Note:  Your Direct Deposit will not take effect for at least 30 days.  (If you do not have a voided check, you must provide a direct deposit authorization form from your bank.)

Wage Deduction Authorization Agreement: Read, complete, sign, date.
Item VI
Designation of Beneficiary:  Read, complete, and WAIT to sign before a notary.  Signature must be before a notary public.
Item VII
Social Security Statement:  Print name, ID # (this is your Social Security #), read, sign and date.
Item VIII
Group Term Life Insurance: Complete, sign and date.  Employment date is your first duty day.
Item IX
Texas New Hire Reporting: Complete Employee Information section (Items 14-22) only.
Item X
Employee Handbook Receipt:  Read, print name, sign and date.
Item XI
Employee Agreement Electronic Communications: Print and read all board policies and keep them for your home records.
Acceptable Use Agreement: Read, print name, print campus/department, sign and date.
Item XII
Drug-Free Workplace Requirements: Read, sign and date.
Item XIII
Employee Directory Information: Read, select option, sign and date.
Item XIV
District Policies--Harassment: Print and read all board policies and keep them for your home records.
Harassment Receipt and Acknowledgement: Read, print name, sign and date.
Item XV
Fair Labor Standards Act Acknowledgement: (Only for paraprofessionals, secretaries, technical staff, aides, clerical staff, transportation, police officers, food service workers, custodians, and maintenance workers) Read, sign and date, detach pages 2-5 for your home records.
Item XVI
Employee Benefits: New hires are eligible for benefits the first day of the following month when employment begins.  It is MANDATORY to enroll or decline online no later than 31 days after you begin employment.  (If you are hired for the new school year, you will enroll during the annual enrollment period at the Benebridge website.)  If you have any questions, please contact the Benefits Office at (210) 945-6216.
