Information for Substitutes
Welcome to the Schertz-Cibolo-Universal City Independent School District. The Human Resources Department would like all new SCUCISD Substitutes to print and complete all forms listed below, in order to expedite processing during the Substitute Orientation sessions. Please begin by printing the Substitute Forms Checklist, and use this as a guide for completing the remainder of your paperwork.
It is important to keep your forms in the order listed here. If you have any questions on filling out a particular form, please leave it blank and we will help you complete the form at orientation.

Form I-9: Complete Page 3, Section 1, sign and date. A copy of your social security card and driver's license is required. |

Form W-4: Complete Items#1-7, sign and date. |

Electronic Funds: Complete, sign, date and attach a voided check for the direct deposit of your pay monthly. Note: Your Direct Deposit will not take effect for at least 30 days. (If you do not have a voided check, you must provide a direct deposit authorization form from your bank.) |

Reference Forms: Read cover page, print name and signature on each reference form, and have two forms completed. The forms can be returned directly by the applicant or by his/her references. You need to get references from any appropriate supervisor. If you have not worked in a business type environment or have been self-employed in the past five years, get references that attest to your character, integrity, and ability to work with students.
