Information for Substitutes

Welcome to the Schertz-Cibolo-Universal City Independent School District.  The Human Resources Department would like all new SCUCISD Substitutes to print and complete all forms listed below, in order to expedite processing during the Substitute Orientation sessions.  Please begin by printing the Substitute Forms Checklist, and use this as a guide for completing the remainder of your paperwork.

It is important to keep your forms in the order listed here.  If you have any questions on filling out a particular form, please leave it blank and we will help you complete the form at orientation.

Important Documents
Item I
Substitute Application: Complete all portions of the Substitute Application.
Item II
Substitute Information: Complete all portions of the Substitute Information form.
Item III
Form I-9: Complete Page 3, Section 1, sign and date.  A copy of your social security card and driver's license is required.
Item IV
Form W-4: Complete Items#1-7, sign and date.
Item V
Electronic Funds: Complete, sign, date and attach a voided check for the direct deposit of your pay monthly.  Note:  Your Direct Deposit will not take effect for at least 30 days.  (If you do not have a voided check, you must provide a direct deposit authorization form from your bank.)
Item VI
Acknowledgement and Designation of Beneficiary Form: Complete all portions of the form, sign and date.
Item VII
Texas New Hire Reporting: Complete Employee Information section (Items 14-22) only.
Item VIII
Social Security Statement:  Print name, ID # (this is your Social Security #), read, sign and date.
Item IX
Employee Handbook Receipt:  Read, print name, sign and date.
Item X
Employee Agreement Electronic Communications: Print and read all board policies and keep them for your home records.
Acceptable Use Agreement: Read, print name, print campus/department, sign and date.
Item XI
Drug-Free Workplace Requirements: Read, sign and date.
Item XII
District Policies--Harassment: Print and read all board policies and keep them for your home records.
Harassment Receipt and Acknowledgement: Read, print name, sign and date.
Item XIII
The Premier Plan General Information: Print and read The Premier Plan Advantage Information and Highlights and keep them for your home records.
Deferred Compensation Plan:  Print, read, and sign and date page 2.
Item XIV
Fingerprint Authorization: Read, print name, sign and date.
Item XV
Reference Forms: Read cover page, print name and signature on each reference form, and have two forms completed.  The forms can be returned directly by the applicant or by his/her references.  You need to get references from any appropriate supervisor.  If you have not worked in a business type environment or have been self-employed in the past five years, get references that attest to your character, integrity, and ability to work with students.
